FaxMyDoctor is operated by [Your Business LLC] ("we", "us"). By sending a fax through FaxMyDoctor, you agree to these terms.

The service

You upload a document, give us a US or Canadian fax number, and pay. We transmit the document to that number and email you a confirmation when it's delivered. No account is required.

Your responsibilities

• You have the right to send the document and to send it to that recipient.
• You won't send unsolicited advertisements. Junk faxes are illegal under the federal Telephone Consumer Protection Act, and we limit each order to a single recipient.
• You won't send anything illegal, threatening, harassing, or that infringes someone else's rights.
• The fax number you enter is correct. We send to exactly the number you give us.

We may refuse or cancel any order we believe breaks these rules.

Health information

FaxMyDoctor is for patients and caregivers. You may send your own health information, or a family member's or someone else's if you have their permission to do so. Before each fax, you consent to us processing the document, which may contain health information, only to send it.

FaxMyDoctor is not a healthcare provider and does not act on behalf of your doctor or any healthcare provider or health plan. It is not intended for healthcare providers, health plans, or their business associates sending information on behalf of patients, and we do not sign Business Associate Agreements.

Delivery and refunds

If a fax can't be delivered (for example, the line is busy or doesn't answer), we retry up to 3 times in total. If it still can't be delivered, we refund you in full automatically. See our Refund Policy.

"Delivered" means the receiving fax machine or service confirmed receipt. We can't control what happens to the fax after that, such as whether the recipient reads it or acts on it.

Your documents

We don't keep your documents. They're deleted as soon as your fax is delivered or refunded, and unpaid uploads are deleted within an hour. See our Privacy Policy. Keep your own copy; we can't resend or recover a document after it's deleted.

Disclaimers and liability

The service is provided "as is." Fax transmission depends on phone networks and receiving equipment we don't control, so we can't guarantee that every fax will be delivered or delivered by a particular time. To the fullest extent permitted by law, our total liability for any claim relating to an order is limited to the amount you paid for that order, and we aren't liable for indirect or consequential losses, such as a missed deadline.

Changes and governing law

We may update these terms; the version in effect when you place an order applies to that order. These terms are governed by the laws of [Your State].
